The Hospice at Home team provides expert nursing care in patients' own homes

Hospice at Home nurses operate seven days a week, in partnership with Primary Care services, and visits from the medical team can be arranged if required.

The team can support you and your family by easing symptoms such as pain and sickness, helping to relieve any worries or fears, and keeping you comfortable and safe in your home environment. We also offer short-term respite care and a night-sitting service.

Who are Hospice at Home?

Care is provided by a team of nurses and healthcare assistants, who provide nursing and personal care. The team also provides essential emotional support for you and your family.

Carers also receive care too, meaning they have time for themselves and may go out for a while. This can ease the tension and anxiety of a carer’s situation, allowing them to be the sister, brother, husband or wife and friend they are once more.
