Specialist Palliative Care Referral Form

The Specialist Palliative Care Team (SPCT), is a group of health care professionals who provide specialised care and support to patients with life-limiting illness and their families in their own home, alternative care settings and the hospice Inpatient Unit. They work collaboratively to address the physical, emotional, social, and spiritual needs of patients, aiming to improve their quality of life and provide comfort and dignity during the end of life stage. The team also works closely with primary care providers and other health care professionals to ensure a comprehensive and coordinated approach to care.

If the patient is in receipt of holistic care and support from the primary care team and symptoms are stable, it may not be appropriate for the SPCT to offer specialised support. Before making a referral please take a look at our referral criteria document (below) for guidance.

Learning and Development

Our education programme is designed to meet the needs of staff working in community health and social care settings.

We have training facilities on-site and a team of experienced trainers who are palliative care specialists. Courses can be booked by selecting the course below and filling in our booking form.

Staff development

We pride ourselves on delivering specialist care to our patients and their families. We are also passionate about influencing end of life care in all settings.

Throughout the year education and training sessions are delivered on topics relevant to staff. Clinical staff complete mandatory training, which includes updating and refreshing clinical skills. This is supported by online learning and a range of training events relevant to delivering safe and effective clinical care.
