Support at home
Most of our support is provided to people living in their own homes and in care or nursing homes.
Our aim is to enable patients to live well at the end of their life. We will work with you to identify what is important to you, set goals and give you the tools you need to achieve them.
Our multidisciplinary team includes doctors, nurses, healthcare assistants, physiotherapists, occupational therapists, complementary therapists, counsellors, socials workers and a spiritual support lead. The whole team works together to support patients at home.
Our support is offered in a range of ways:
- Appointments at the hospice. We may invite you in for an appointment with a member of our team (such as a doctor or nurse), for a group session through our Living Well service, or for support from our therapies, counselling, social work or spiritual support teams.
- 24/7 Hospice Line. Our telephone advice line is available 24-hours a day, seven-days a week to answer any questions you might have about symptoms, medication or anything else that is worrying you. They will be able to offer advice, or refer you to the right team for support.
- Home visits. If you are unable to visit the hospice, our team may arrange a visit with you at home to provide help, advice, care or support. This might include Advance Care Planning or help managing your symptoms.
As a specialist palliative care service, we work closely with your GP, hospital team and other NHS services. We do not replace them, but we can offer advice and support them to support you.
Our home visits are always planned. If you need urgent help with symptoms, or help out-of-hours, then other providers will visit to support you. You can contact our 24/7 Hospice Line who can give advice and will work with the NHS and other providers if an urgent visit to your home needs to be organised.
Personal care
Our care does not include hands-on personal care for things such as washing or dressing. We can help you to access this type of care, and we can help you get funding for it if you are eligible.

We ask all staff, volunteers, patients, family members and visitors to please:
- Be kind.
- Be respectful.
- Be compassionate.
- Be polite.
- Be mindful of others.
- Protect people’s dignity.
Poor conduct towards others is never ok. Our staff and volunteers may end an appointment or group session early if anyone is aggressive, abusive, violent or threatening. If an incident happens on hospice premises, the person may be also be asked to leave.
If you feel that your particular needs have not been met we’d like the chance to sort it out. Please speak to a member of the team and we will do our best to support you appropriately.
