Living Well
Living Well is an approach facilitated by St Wilfrid’s Hospice that provides a range of support to empower our patients, their families and the wider community. This helps us to evolve the hospice’s mission for our community to live well at the end of life. This support takes many forms, such as clinical interventions or treatment, education groups, providing social connections and linking with community groups and organisations. This could take place in the Living Well Hub and other areas of the hospice or through external links. Using feedback from personal experience we will continue to focus on person-centred support to inform our future planning.

Welcome to our Living Well Hub!
Situated at the hospice, this welcoming space is available for one-to-one patient appointments in our treatment rooms, group activity in our Garden Room and classes and individual therapy appointments in our Gym.
It’s also home to our Community Links network information resource. This is where we display information and leaflets from external organisations that offer a variety of support and activities available in our catchment area.
Linking with community groups and organisations
Through our Community Link Network, we establish connections with groups and organisations that may offer services and activities that could be of interest to our patients, carers and those who have been bereaved.

Monday
Fatigue and A 4-week rolling group (FAB group) - 3-4pm - The Garden Room
A 4-week rolling programme aimed at supporting those struggling with fatigue and/or breathlessness self-manage their symptoms. The aim is to provide people with a toolkit of strategies to empower them to remain active and independent with everyday activities. Referral only.
Tuesday
Living Well Hub Drop-in - 2-4pm - The Street Café
An opportunity for patients and carers to meet our drop-in team in a relaxed and informal environment. The drop-in offers Nurse-led and Therapy-led clinics and an opportunity to learn about further support in the community. A weekly, sociable event where you can meet other people who may be facing similar issues. Weekly Drop-in.
Carers group - 2.30-3.30pm - Living well Garden room
A drop-in carers group for palliative patient carers that is held once a week in the Living Well Garden room from 2.30-3.30 PM every Tuesday.
This is a support group that is designed to provide a safe and welcoming space for carers who are taking care of their loved ones with a life limiting illness.
The group is open to anyone who is caring for a patient known to the hospice, whether they are a family member, friend or professional care giver.
The carers group will be facilitated by a volunteer with professional input from our carers social worker, who has experience working with palliative patients and their carers. The facilitator will provide emotional support and practical advice to carers as they navigate the challenges of the caring role. During the group meetings carers will have the opportunity to share experiences, ask questions and receive support from their peers. The facilitator may also lead discussion and offer help.
The drop-in carers group is an invaluable resource for carers who are facing the emotional and physical challenges of caring for a loved one at the end of their life. It provides a supportive community that helps carers feel less isolated and more empowered to provide the best care possible for their loved ones.
Wednesday
Patient Support Group - 2-4pm - The Garden Room
An informal meeting place for individuals known to the Hospice to meet every week for gentle respite. We focus on being welcoming and inspirational in a safe and supportive environment. The group enables patients to discuss what matters to them whilst exploring and sharing their personal experiences.
Weekly Drop-in.
Gym Group - 12.30-1.30pm - The Gym
These sessions are run by a volunteer with experience in physiotherapy and rehabilitation. The aim of the class is to provide weekly exercise to help maintain a current level of mobility, fitness and quality of life. Referral only.
Tea at Three - First Wednesday of every month - 2-4pm - The Street Café
An informal support group and welcoming space for anyone who has experienced a bereavement of a loved one who was cared for by St Wilfrid’s Hospice. The Group is facilitated by volunteers and offers a supportive and caring environment to openly discuss thoughts and feelings.
Drop-in. Open to bereaved relatives and carers of St Wilfrid’s patients.
Thrive - 10.30-11.30am - The Gym
The name is in the title! Thrive. This is an 8-week circuit-based exercise programme aimed at improving function and quality of life.
Referral only, St Wilfrid’s patients only.
Thursday
Living Well Matters Group - 2-3pm - The Garden Room
This is a 7-week programme which introduces patients to St Wilfrid’s, the services we provide and the members of the Multi-Disciplinary Team you’ll meet along the way. It’s a gentle introduction to The Hospice support that patients may need to access during their palliative journey as well as an update about what is potentially available in the community from statutory services.

Friday
Relaxation Group - 2-3pm - Zoom (with audio)
This weekly group is from the comfort of your own home and is open to patients, carers and those who are bereaved. The group practises various techniques to help with relaxation, including breathing techniques, progressive muscle relaxation, mindfulness meditations and guided visualisations. This relaxation can assist with breathlessness, anxiety, stress, tension, insomnia and pain as well as improving quality of life and wellbeing.
