Our vision for 'Closer to You'

One third of people
who would benefit from hospice care are not currently accessing the care that they need. Closer to You is a project designed to help people who need support to live well until the end of their lives. Our organisational goal is to make sure that no-one dies alone, afraid or in pain.

Hospice care can help people live well until the end of their lives, but one third of those who would benefit from St Wilfrid’s support are not currently accessing our services.

St Wilfrid’s Hospice provides palliative and end of life care to adults with life-limiting illnesses in Eastbourne, Seaford, Hailsham, Uckfield, Heathfield and Pevensey and in the rural areas in between and beyond.

We support people in our 20 bedded In Patient Unit, our Wellbeing Centre and in their own homes. Our care has been rated as ‘Outstanding’ by the Care Quality Commission and, when we are involved, patients are four times less likely to die in hospital and twice as likely to be able to die in their own homes.

Of the c.3,000 people who die within our catchment area each year it is estimated that 2250 would benefit from specialist end of life care. At the current time, St Wilfrid’s are supporting around two thirds of this number with the vital help of other local services.

Through our ‘Closer to You’ campaign, we plan to expand our care to reach more people through:

Reaching more people in the towns and villages across East Sussex. Of the 235,000 people living within our catchment area, around half live in small towns and rural villages, but are currently under-represented within our patient base.

Making care accessible to elderly people. Eastbourne, Hailsham and Seaford CCG has the highest proportion of people in England over 75, with over half the patients supported by St Wilfrid’s over 75 and a quarter over 85. Care provided in a local setting would make it easier for our elderly population and those with limited mobility to access our services.

Supporting people with both cancer and non-cancer diagnoses. Two thirds of the people who die within our catchment area have a non-cancer diagnosis, but only 25% of St Wilfrid’s patients are people with non-malignant illness. People with non-cancer diagnoses, including cardiovascular disease (39% of deaths in our catchment area), respiratory disease (15% of deaths), motor neurone disease and dementia, can benefit greatly from hospice care, and, through Closer to You, we plan to reach more people in these groups.

Increasing awareness of the care that is available. Many people who would benefit from hospice support are unaware of the range of treatments and therapies which could improve their quality of life at an earlier stage of their illness, and which could  help to keep them independent for as long as possible. Through Closer to You we will increase awareness of the care on offer to both cancer and non-cancer patients, and of the range of support which is available to the whole family, before and after bereavement.

Reducing loneliness and isolation.  When a member of the family is seriously ill, both the patient and their carers can become isolated, particularly when the family is elderly or less mobile and living in a rural setting.  Through Closer to You we aim to expand our volunteer network, providing befriending and social support.
