Hospice at home

Some people prefer to stay at home – our Hospice at Home team visit them to provide necessary care and support.

Patients are referred to us by GPs, Hospital Consultants, District Nurses and Macmillan Clinical Nurse Specialists - we work together to provide specialist and supportive palliative care to patients and their families.

We aim to support and empower patients and their families in choices they make regarding their care during their illness and at end of life. Currently, we visit patients every day of the week 8.30am - 4.30pm.

Examples of care and support that we provide in patients' homes and nursing homes:

  • Hands-on nursing care
  • Psychological and spiritual support
  • Carer support
  • Respite care
  • Short-term respite in local nursing homes
Caring for the communities of Eastbourne, Polegate, Pevensey, Seaford, Hailsham, Heathfield, Uckfield and everywhere in between
