What you want to share will be personal to you, but may include the name of a person you are happy to be consulted on your behalf at a later time, your preferred place of care, your thoughts on different treatments or types of care that might be offered, or solutions to practical issues, such as who will look after your pet should you become too unwell. You can include any wishes that are important to you, no matter how trivial they may seem.
Advance care planning is a process of discussion between you and those involved in your care. This may mean talking to our doctors and nurses, a care home manager, social worker, or your family and friends.
Whether or not you choose to plan for your future in this way is entirely up to you. If you would like to go ahead, we will be happy to offer advice and support to you and your family as you explore your options. We understand that your decisions may change too: we can revisit your plan at any time.
