Patients are referred to us by GPs, Hospital Consultants, District Nurses and Macmillan Clinical Nurse Specialists.

As an organisation working separately from the NHS, we require detailed information at the time of referral. 

We have a shared referral pattern for our services with the Primary Care Trust based Macmillan Clinical Nurse Specialist service.  Our combined referral form can be downloaded here and faxed or posted to us.  If you are unsure of the service that is most required, it may be that you request for us to triage the patient to the most appropriate service. 

For urgent referrals, we will do our best to meet the need within one working day, but please call us to highlight that an urgent referral has been faxed or posted to us. Where patients are known to the Specialist Palliative Care Service, either Community Macmillan Nurse or hospital services, additional requests for admissions can be requested on a simpler form.

Patients who have been discharged from our service can be readmitted when necessary through a repeat referral form, telephone call, or letter.
