Professional development and partnerships with other clinical professionals is deemed of high importance here at St Wilfrid’s Hospice.

Patients are referred to us by GPs, Hospital Consultants, District Nurses and Macmillan Clinical Nurse Specialists. As a specialist provider, we seek to work to the best of our abilities in partnership with all health care professionals providing palliative and end of life care.  During a patients journey, the need for specialist involvement will alter and at no time are the services of the Hospice meant to take over from our Primary Care colleagues.

Our aim is to empower our colleagues and give them the tools to provide the best possible care when it is most needed.  Our Hospice at Home team in particular, will always work in a close partnership with District Nurses and social care agencies.  The Macmillan Clinical Nurse Specialist team is currently based in the Primary Care Trust but we have an extremely close working relationship, sharing clinical knowledge and resources.
